 Introduction: Better life conditions, and consequently living along life leads to old age. At present time %25 of developed communities and %4 of our country are aged. As we know giving efficient services for every specific population needs being familiar, Recognizing needs and health care problems, etc of this specific group. Since frail elderlies are a susceptible group therefore, we must completely assess their needs in order to provide correct services.  Methods & Material: It wasdescriptive and cross sectional study to assess health care needs of the frail elderlies dwelled in 13th district of Tehran and theircare givers’ problems in Boali hospital and Khayam center 50 people of them that possessed entrance criteria formed the study population. The data were gathered via questioners and processed by SPSS. Results: Results showed that their mean age was 72.9 years, %76 of female frail elderlies were illiterate and %84 of them had hospitalization history in last years. The most common reason for their first time hospitalization was cardiac diseases (%50). %60 of female frail elderlies and %44 of male frail elderlies used more than 3 types of drugs. Among the most important concerns: %74 of frail elderly were health and hygiene. There were a significant relationship between mental status of male and female frail elderlies (p=0.004). Most care givers (%64) were between 30-55 years old group and %90 of them were married and had children. Because of economical problems all informal care givers avoided employing formal care givers. Conclusion: The study results indicated that the frail elderlies were severely vulnerable for different damages and diseases and needed continuous care and care givers had more problems. Providing good support not only will lead to home residence but also will prevent the complications of staying in age centers, and care givers will find some opportunities for loneliness and paying attention to their own family membersandwill be far from the negative effects of taking elderlies to aged centers.
