تعیین تأثیر مداخلات سبک زندگی بر شدت علائم نوروپاتی محیطی بیماران مراجعهکننده به انجمن دیابت ارومیه در سال 1392

پیشزمینه و هدف: یکی از بزرگترین چالشهایی که در قرن بیست و یکم سیستمهای بهداشتی در سراسر جهان با آن مواجه میباشند، افزایش بار ناشی از بیماریهای مزمن است. سبک زندگی نامناسب و غیربهداشتی مسئول وقوع بسیاری از انواع بیماریهای مزمن است. دیابت ملیتوس بیماری خطرناک و مشکل عمده و مهم در کشورهای درحالتوسعه میباشد. دیابت و مخصوصاً عوارض ناشی از آن میتواند اثرات منفی و عوارض نامطلوب زیادی بر روی کیفیت زندگی سلامتمحور انسانها بر جای گذارد. نوروپاتی یک عارضه شایع و خطرناک دیابت است که بیشترین هزینه مراقبتی را در اینگونه بیماران ایجاد مینماید و از عوارض میکروواسکولار بوده و رایجترین مشکل مزمن بیماران مبتلا به دیابت میباشد و در بعضی از بیماران همراه با درد نوروپاتیک است. با تغییر در سبک زندگی میتوان تا 90 درصد از عوارض دیابت نوع 2 پیشگیری کرد. این مطالعه با هدف تعیین تأثیر مداخلات سبک زندگی بر شدت علائم نوروپاتی محیطی دیابت انجام گرفته است.
مواد و روشها: این پژوهش یک مطالعه از نوع کارآزمایی بالینی است. ابزار گردآوری دادهها شامل یک پرسشنامه بررسی اطلاعات جمعیتشناسی، یک چکلیست جهت قند خون و پرسشنامه معیار تورنتو میباشد. جهت تجزیه تحلیل اطلاعات نیز از آمارهای توصیفی و آزمون t در محیط نرمافزار SPSS نسخه 16 استفاده شد و اختلاف در سطح کمتر از 05/0 معنیدار تلقی گردید.
یافتهها: درمجموع تعداد 74 نفر در این مطالعه شرکت کردند (37 نفر گروه کنترل، 37 نفر گروه مداخله). علائم و شدت نوروپاتی در افراد گروه مداخله بهطور معنیداری بهبود یافته بود (001/0 P
Background & Aim: Diabetic peripheral neuropathy likely affects up to one-third of adults with diabetes. All diabetic patients are likely to develop peripheral neuropathy if they live sufficiently long. Drug therapy has limited potential for controlling the associated pain. Alternative methods of treatment have thus far demonstrated limited success, so the aim of study was evaluating the effect of lifestyle interventions on diabetic neuropathy severity among diabetic outpatients that suffering diabetic neuropathy.
Material & Methods: This clinical trial was performed as time series on 76 patients with diabetic neuropathy, in Urmia in the year of 2013. After matching the study variables, the patients were divided equally and with random allocation to two experiment and control groups. The lifestyle interventions were performed for the experiment group beginning 4 educative sessions on diabetes self- care that emphasizes strategies for glucose monitoring, dietary and exercise habits, foot care, medication taking and coping. Each session was lasted for1.5 hours.Then, these patients were followed for 12 weeks. During these 12 weeks, they were given counseling on lifestyle interventions. Diabetic neuropathy symptom severity in both groups was measured by using Toronto Clinical Neuropathy Score at the beginning of study and then at the end of counseling for 12 weeks. The data were analyzed by SPSS 16.0 software using t-test .
Results: There wasn’t any difference between the mean of diabetic neuropathy symptom severity in both study groups before the intervention, but comparing the mean of diabetic neuropathy symptom severity after intervention, there was a significant difference (p
