en بررسی تأثیر مراقبت‌های استاندارد راه هوایی مصنوعی بر پنومونی وابسته به ونتیلاتور مدیریت پرستاری Nursing management پژوهشي Research تاریخ دریافت 25/06/1394 تاریخ پذیرش 30/08/1394 پیش‌زمینه و هدف: مراقبت‌های استاندارد راه هوایی می‌تواند منجر به کاهش بروز پنومونی وابسته به ونتیلاتور شود. این مطالعه باهدف بررسی تأثیر مراقبت‌های استاندارد راه هوایی مصنوعی بر پنومونی وابسته به ونتیلاتور انجام شد. مواد و روش‌ها: در این مطالعه کارآزمایی بالینی، 64 بیمار بستری در بخش‌های مراقبت ویژه بیمارستان الزهرا اصفهان که تحت تهویه مکانیکی بودند به روش آسان انتخاب شدند. سپس نمونه‌ها به‌صورت تصادفی به دو گروه مداخله و کنترل تخصیص داده شدند. در گروه مداخله مراقبت‌های استاندارد راه هوایی مصنوعی و در گروه کنترل مراقبت‌های روتین انجام شد. مقیاس بالینی عفونت پولمونر تعدیل‌شده در مراحل قبل و روزهای سوم، چهارم و پنجم مطالعه برای تمام نمونه‌ها تکمیل شد. یافته‌ها: نتایج این مطالعه نشان داد که تا روز چهارم مطالعه، میانگین نمره مقیاس بالینی عفونت پولمونر تعدیل‌شده در هر دو گروه مداخله و کنترل مشابه بود؛ اما در روز پنجم مطالعه میانگین نمره مقیاس بالینی عفونت پولمونر تعدیل‌شده در گروه مداخله به‌طور معنی‌داری کم‌تر از گروه کنترل بود. بحث و نتیجه‌گیری: نتایج این مطالعه نشان داد که در بیماران تحت تهویه مکانیکی، مراقبت‌های استاندارد راه هوایی مصنوعی می‌تواند منجر به افزایش کم‌تری در میانگین نمره مقیاس بالینی عفونت پولمونر تعدیل‌شده بیماران شود. لذا جهت پیشگیری از پنومونی وابسته به ونتیلاتور به پرستاران توصیه می‌شود که این مراقبت‌ها را انجام دهند. Received: 16 Sep, 2015 Accepted: 21 Nov, 2015 Background & Aims: Artificial airway standardization cares could reduce the incidence of Ventilator Associated Pneumonia (VAP). This study aimed to evaluate the effect of an artificial airway standardization cares on the VAP. Materials & Methods: In this clinical trial, 64 patients were selected among those who had undergone mechanical ventilation in the ICUs of Al-Zahra Hospital, Isfahan, Iran using convenience sampling method. Then, the subjects were randomly allocated to intervention and control groups. In the intervention group the artificial airway standardization cares and in the control group the routine cares were done. Modified Clinical Pulmonary Infection Score (MCPIS) was completed for all the subjects in the stages before, third, fourth and fifth day of study. Results: The results of this study showed that until fourth days of study, the mean score of MCPIS were similar in both intervention and control groups. But on the fifth day of study the mean score of MCPIS in the intervention group was significantly lower than control group. Therefore, in order to prevent VAP, the nurses are recommended to perform this cares. Conclusions: The results of this study showed that in patients undergoing mechanical ventilation, the artificial airway standardization cares could lead to the lower increasing in the mean score of MCPIS.
