en ارتباط تعهد اخلاقی با خطاهای دارویی اظهارشده در پرستاران شاغل در مراکز آموزشی درمانی دانشگاه علوم پزشکی ارومیه، سال 1397 THE RELATIONSHIP BETWEEN ETHICAL COMMITMENT AND MEDICATION ERROR AMONG NURSES IN CENTERS AFFILIATED TO URMIA UNIVERSITY OF MEDICAL SCIENCES IN 2018- 9 پرستاری Nursing پژوهشي Research پیش‌زمینه و هدف: خطاهای دارویی رایج‌ترین خطای پزشکی است که به‌عنوان یکی از عناصر دخیل در ایمنی بیمار بشمار می‌آید و عوارضی را ایجاد کند که در اثر ترکیب دو عامل انسانی و عامل سیستمی ایجاد می‌شود. تعهد اخلاقی پرستاران از متغیرهای مهم رفتار فردی و سازمانی است که بر عملکرد شغلی و رفتار آنان در خوداظهاری خطاهای دارویی اثرگذار می‌باشد؛ مطالعه حاضر باهدف تعیین ارتباط تعهد اخلاقی با خطاهای دارویی اظهارشده در پرستاران مراکز آموزشی درمانی دانشگاه علوم پزشکی ارومیه انجام شد. مواد و روش‌ها: مطالعه حاضر یک مطالعه توصیفی همبستگی است که 326 پرستار شاغل در پنج مرکز آموزشی درمانی ارومیه با روش نمونه‌گیری مرحله‌ای و با استفاده از دو پرسشنامه تعهد اخلاقی کادوزیر و خطاهای دارویی اظهارشده محقق ساخته مورد برسی قرار گرفتند. درنهایت پس از جمع‌آوری داده‌ها؛ به‌وسیله نرم‌افزار SPSS18 با استفاده از آمارهای توصیفی و تحلیلی مورد تجزیه‌وتحلیل قرار گرفت. یافته‌ها: نمونه‌ها اکثراً مؤنث (8/71درصد) با مدرک کارشناسی (7/88درصد) بودند. میانگین وقوع خطای دارویی در سه‌ماهه گذشته برابر 64/1±1/1 برای هر پرستار بود. بیشترین نوع خطا؛ دادن دوز اشتباه، ثبت اشتباه و داروی اشتباه بود. 5/29 درصد خطاها ازنظر اهمیت در سطح متوسط و بالا بودند که 7/26 درصد منجر به آسیب در بیماران شده بود. ارتباط معنی‌دار بین سابقه کار و میزان خطا مشاهده شد. اکثر پرستاران نمره تعهد اخلاقی پایین (میانگین 6/7± 67/30) را کسب کردند. بین تعهد اخلاقی و نمره خطای دارویی همبستگی معنی‌داری یافت نشد (035/0 =r، 52/0=p). بحث و نتیجه‌گیری: باوجود عدم ارتباط معنادار بین خطاهای دارویی و تعهد اخلاقی به‌عنوان یک عامل فردی، به نظر می‌رسد نقش عوامل سازمانی در ایجاد خطاهای دارویی پررنگ‌تر می‌باشد. در کنار اهتمام بیشتر به رفع عوامل فردی در جهت کاهش خطاهای دارویی، ضرورت دارد که مدیران و مسؤولین بهداشتی در جهت ارتقاء و بهبود عوامل سازمانی در جهت کاهش خطای دارویی و افزایش ایمنی بیمار تلاش نمایند. Background & Aims: Medication error is the most common medical error and it is one of the factors involved in patient safety and it can create complications caused by the combination of both human and systemic factors. The ethical commitment of nursing staff is one of the important variables of organizational behavior that affect their job performance. The purpose of this study was to determine the relationship between ethical commitment and medication error among nurses working in centers affiliated to Urmia University of Medical Sciences in 2018-9 Materials & Methods: This descriptive correlational study was performed on 326 nurses working in 5 medical centers of Urmia. Subjects were selected by multistage sampling method. Data were collected using the Cadozier's ethical commitment questionnaire and the researcher-made questionnaire of self-reported medication errors. Finally, after data collection, data were analyzed by SPSS18 software using descriptive and analytical statistics. Results: In this study, most of the samples were female (71.8%) with a bachelor's degree (88.7%). The mean incidence of medication error in the last three months was 1.64 ± 1.1 per nurse. The most common type of error was the wrong drug with the wrong dosage and incorrect documentation. 29.5% of the errors occurred at moderate to high levels of significance, and 26.7% of the errors resulted in injury to patients. There was a significant relationship between work experience and error rate. The majority of nurses had a low ethical commitment score (30.67 ± 7.6). No significant correlation was found between ethical commitment and error score (frequency of errors)(p = 0.52, r = 0.035). Conclusion: Although there was no significant relationship between medication error and ethical commitment as an individual factor, the role of organizational factors in medication errors seems to be important. It is essential to focus on individual factors to eliminate medication errors.
