بررسی میزان تبعیت از پروتکل کشوری درمان بیماری تب مالت توسط پزشکان استان همدان
EVALUATION OF THE RATE OF ADHERENCE TO THE NATIONAL PROTOCOL FOR THE TREATMENT OF BRUCELLOSIS BY PHYSICIANS OF HAMADAN PROVINCE

پیشزمینه و هدف: بروسلوز یکی از مهمترین بیماریهای مشترک بین انسان و دام است که از جنبه اقتصادی، بهداشت عمومی و درمانی موردتوجه ویژه است. در حال حاضر یکی از مشکلات مهم در بیماری بروسلوز میزان نسبتاً بالای عود و موارد مزمن بیماری میباشد که ممکن است به دلیل روشهای درمانی متفاوت در این بیماران باشد. این مطالعه باهدف تعیین میزان تبعیت پزشکان از پروتکل کشوری درمان تب مالت انجام شد.
مواد و روش کار: مطالعه حاضر بهصورت مقطعی در استان همدان در سال 1400 بر روی 5524 نفر که در فاصله سالهای 98-1394 به بیماری تب مالت مبتلا بودهاند و توسط مراکز بهداشت شهرستانهای استان همدان پیگیری شده و فرم بررسی اپیدمیولوژیک برای آنها تکمیلشده بود انجام گردید. دادهها با استفاده از نرمافزار آماری 24 SPSS با استفاده از آزمونهای تی تست، و آزمون کای اسکوئر یا تست دقیق فیشر آنالیز آماری شد.
یافته ها: از 5524 بیمار بررسیشده، در بزرگسالان 3450 بیمار (8/66 درصد) رژیمدرمانی 1، 1123 بیمار (7/23 درصد) رژیمدرمانی 2 و 41 نفر (8/0 درصد) رژیم کودکان را دریافت کردند و 115 نفر از کودکان (8/31 درصد) رژیمدرمانی 1 بزرگسالان، 50 نفر (8/13 درصد) رژیمدرمانی 2 بزرگسالان و 21 نفر (8/5 درصد) رژیمدرمانی کودکان را دریافت کردند. 82 درصد بزرگسالان و 37 درصد کودکان طبق پروتکل، و در مقابل 18 درصد بزرگسالان و 63 درصد کودکان مغایر با دستورالعمل کشوری دارو دریافت کرده بودند.
بحث و نتیجه گیری: یافتههای مطالعه حاضر نشان داد وضعیت تبعیت و اجرای پروتکل کشوری در سطح استان همدان مطلوب نیست و ضروری است جهت بهبود وضعیت موجود، برنامههای آموزشی و مداخلهای مدنظر مسؤولین قرار گیرد.
Background & Aim: Brucellosis is one of the most important diseases between humans and animals that has been specially considered in terms of economics, public health, and treatment. Currently, a major problem in brucellosis is its relatively high recurrence and high rate of chronic cases, which may be caused by variety of treatment methods for these patients. This study was performed to determine the degree of compliance of the physicians with the National Protocol to treat brucellosis in different population groups in Hamadan province in 2015-2019.
Materials & Methods: This cross-sectional study investigated 5524 patients who had brucellosis between 2015 and 2019 and were followed up by the health centers of the cities of Hamadan province and their epidemiological examination form was completed. Data were analyzed by SPSS 24 software using t-test, Chi-square test, and Fisher's exact test.
Result: Out of 5524 patients, 3450 (66.8%) received treatment diet 1, 1123 (23.7%) received treatment diet 2, and 41 (0.8%) received the pediatric regimen Moreover, 115 children (31.8%) received treatment adult diet 1, 50 (13.8%) received treatment adult diet 2, and 21 paients (5.8%) received the pediatric treatment diet. In addition, 82% of adults and 37% of children received a protocol-based medication but 18% of adults and 63% of children received medication contrary to the national guidelines.
Conclusion: The findings of the present study demonstrated that compliance and implementation of the national protocol have not reached a desired level in the Hamadan province. Therefore, educational and intervention programs should be considered by the respective authorities to improve the current situation.
