fa مشکلات مستندسازی پرونده‌های پزشکی در پیاده‌سازی سیستم پرداخت مبتنی بر گروه‌های تشخیصی مرتبط (DRGs): یک تحقیق کیفی THE PROBLEMS OF DOCUMENTING MEDICAL RECORDS IN THE IMPLEMENTATION OF A PAYMENT SYSTEM BASED ON DIAGNOSIS-RELATED GROUPS (DRG): A QUALITATIVE RESEARCH بهداشت Public Health کیفی Qualitative پیش‌زمینه و هدف: سیستم پرداخت مبتنی بر گروه‌های تشخیصی مرتبط (DRGs) باهدف کنترل هزینه‌های خدمات مراقبت سلامت، افزایش کارایی در مراقبت‌های بستری و بهبود شفافیت در فعالیت‌های بیمارستانی ایجاد گردید. موفقیت این سیستم بر مستندسازی کامل و دقیق پرونده‌های پزشکی و کدگذاری صحیح تشخیص‌ها و اقدامات وابسته است. هدف از انجام این پژوهش، تعیین مشکلات مستندسازی پرونده‌های پزشکی جهت پیاده‌سازی سیستم پرداخت مبتنی بر DRGs بود. مواد و روش کار: این پژوهش یک مطالعه کیفی بود که با استفاده از روش مصاحبه نیم ساختاریافته در سال 1401 انجام یافت. جامعه پژوهش شامل متخصصان مدیریت اطلاعات سلامت و اقتصاد سلامت در دانشگاه علوم پزشکی ارومیه بودند که به‌طور هدفمند انتخاب شدند. تحلیل داده‌ها نیز از طریق تحلیل محتوا با استفاده از نرم‌افزار ATLAS.ti انجام شد. یافته‌ها: از دیدگاه مصاحبه‌شوندگان مهم‌ترین مشکلات مستندسازی پرونده‌های پزشکی در پیاده‌سازی سیستم‌های پرداخت مبتنی بر <a name="_Hlk130143892">DRGs</a>، شامل عدم وجود دستورالعمل‌های مستندسازی بالینی، عدم وجود عناصر اطلاعاتی ضروری، عدم ارزیابی دقیق کیفیت مستندسازی و کدگذاری، کمبود دوره‌های آموزشی مداوم، ناقص بودن مستندسازی، و عدم تعامل کافی بین پزشکان و کدگذاران در بیمارستان بودند. بحث و نتیجه‌گیری: نتایج این تحقیق نشان داد که برخی متغیرهای ضروری برای پیاده‌سازی سیستم پرداخت مبتنی بر DRGs، در پرونده‌های پزشکی و سیستم اطلاعات بیمارستان به‌صورت مجزا و شفاف تعریف نشده است و کیفیت مستندسازی پرونده‌ها و کدگذاری نیز دقیق و کامل نیست. لذا ضروری به نظر می‌رسد که در فرم‌های استاندارد پرونده پزشکی عناصر اطلاعاتی جداگانه‌ای برای تشخیص اصلی، اقدام اصلی و همچنین بیماری همراه و عوارض در نظر گرفته شود. Background & Aims: The payment system based on diagnosis-related groups was created to control the costs of health care services, increase efficiency in inpatient care, and improve transparency in hospital activities. The success of this system depends on complete and accurate documentation of medical records and correct coding of diagnoses and procedures. This research aimed to determine the problems of documenting medical records to implement a payment system based on DRGs. Materials & Methods: This research was a qualitative study that was conducted using the semi-structured interview method in 2022. The research population included experts in health information management and health economics at Urmia University of Medical Sciences, who were selected purposefully. Data analysis was done through content analysis using ATLAS.ti software. Results: From the point of view of the interviewees, the most important problems of documenting medical records in the implementation of DRGs, included the lack of clinical documentation guidelines, the lack of necessary information elements, the lack of accurate evaluation of the quality of documentation, and coding, the lack of continuous training courses, the incompleteness of documentation, and the lack of sufficient interaction between doctors and coders were in the hospital. Conclusion: The results of this research showed that some essential variables for implementing the DRGs system are not defined separately and clearly in the medical records and the hospital information system, and the quality of the documentation and coding is not accurate and complete.
