مقایسه میزان بروز و درجه زخم فشاری در بیماران سیگاری و غیر سیگاری بستری در بخش مراقبتهای ویژه
COMPARISON OF INCIDENCE AND GRADE OF PRESSURE ULCER IN SMOKERS AND NON-SMOKERS ADMITTED TO INTENSIVE CARE UNIT

پیشزمینه و هدف: یکی از مشکلات جدی در بیماران بستری در بخش مراقبتهای ویژه، زخم فشاری است که با عوارض عمدهای نظیر افزایش طول مدت بستری و تأخیر در بهبودی همراه است. مطالعه حاضر باهدف مقایسه بین میزان بروز و درجه زخم فشاری در بیماران سیگاری و غیر سیگاری بستری در بخش مراقبتهای ویژه انجام شده است.
مواد و روشها: این پژوهش یک مطالعه مقطعی از نوع توصیفی-تحلیلی بود که در آن 120 بیمار در دو گروه با استعمال سیگار (60 نفر) و بدون استعمال سیگار (60 نفر) به روش نمونهگیری در دسترس موردمطالعه قرار گرفتند. دادهها با استفاده از پرسشنامه جمعیت شناختی، مقیاس درجهبندی "انجمن ملی مشاوره زخم فشاری"(NPUAP) و ابزار برادن جمعآوری شدند. تجزیهوتحلیل دادهها با استفاده از نسخه 24 نرمافزار SPSS انجام گردید.
یافتهها: میانگین و انحراف معیار میزان بروز زخم فشاری در گروه با استعمال سیگار 01/11 + 13/46 و در گروه بدون استعمال سیگار 2/8 + 03/39 بود و اختلاف معنادار بین میزان بروز در دو گروه مشاهده گردید (05/0 p≤). میانگین و انحراف معیار درجه زخم فشاری در گروه با استعمال سیگار 92/5+ 32/23 و گروه بدون استعمال سیگار 21/1+ 22/14 بوده است. 7/11 درصد از بیماران در گروه سیگاری دارای زخم فشاری درجه 4 بودند اما در گروه غیر سیگاری زخم فشاری درجه 4 مشاهده نشد. اختلاف آماری معنادار بین درجه زخم فشاری در دو گروه مشاهده گردید (05/0 p≤).
نتیجهگیری: نتایج پژوهش نشاندهنده تأثیر مصرف سیگار بر افزایش بروز و درجه زخم فشاری در بیماران بستری میباشد. لذا لازم است کارکنان بخش مراقبتهای ویژه بهمنظور پیشگیری از ایجاد زخمهای فشاری و گسترش شدت درجه زخم در بیماران بستری، به عوامل خطر توجه خاص نموده و بیماران در معرض خطر را شناسایی و تحت پایش قرار دهند.
Background & Aims: Pressure ulcer is a serious problem in patients admitted to the intensive care unit. It is associated with major complications such as prolonged length of stay and delayed recovery. The aim of this study was to compare the incidence and grade of pressure ulcers in smokers and non-smokers admitted to the intensive care unit.
Materials & Methods: This study is an analytical cross-sectional study in which 120 patients in two groups (smokers (60) and non-smokers (60)) were selected by convenience sampling. Data were collected using a demographic questionnaire, information sheet based on the division of the National Pressure Ulcer Advisory Panel (NPUAP), and Braden's grading criteria. Data were analyzed by SPSS v.24 software.
Results: The results showed that incidence of pressure ulcers in the smoker group was 46.13 ± 11.01 and in the non-smokers group was 39.2 ± 8.23 and there was a significant difference between the incidence of pressure ulcers in two groups (p≤.0/05). Also, comparing the grade of pressure ulcer, the mean and standard deviation of the smoker group was 23.32 ± 5.92 and the non-smokers group was 14.22 ± 1.21. 11/7% of patients in the smoking group had grades of four pressure ulcers, but in the non-smoking group, none of the patients had grades of four pressure ulcers. The t-test showed a significant difference between the grades of pressure ulcers in two groups (p≤0/05).
Conclusion: The results of this study showed the effect of smoking on increasing incidence and grade of pressure ulcers in patients admitted to the intensive care unit. Therefore, it is necessary for the staff of the intensive care units to pay special attention to risk factors and to identify and monitor the hospitalized patients.
